Connect the Doc Privacy Policy

This Privacy Policy explains how Connect the Doc Marketing Inc. (“CTD”) collects, uses, and discloses your personal information while you use CTD's Website (the “Site”). This Privacy Policy is only applicable to this Site and not to any other websites you may access from this Site, each of which may have policies that materially differ from this Privacy Policy.

Consent

By using the Site, you signify your consent to our collection, use and disclosure of your personal information, and all interactions with the Site in accordance with this Privacy Policy. If you do not agree with this Privacy Policy, please do not use the Site.

Information Automatically Collected

1. Non-Identifiable Information – As is typical with many sites, the Site server automatically collects certain non-identifiable information regarding the Site users, such as, but not limited to, the Internet Protocol (“IP”) address of your computer, the IP address of your Service Provider, the date and time you access the Site, the Internet address of the site from which you linked directly to the Site, the operating system you are using, the type of web browser you are using, the sections of the Site you visit, the Site pages accessed and images viewed, and any content you post to or download from the Site. CTD uses this non-identifiable information for Site and system administration purposes. CTD may disclose such non-identifiable information to others and archive it for future use.

2. Cookies – The Site uses “cookies,” a technology that installs information on the Site user's computer to permit the Site to recognize future visits by that computer. Cookies enhance the convenience and use of the Site. For example, CTD uses the information provided through cookies to recognize you as a previous user of the Site, to offer personalized webpage content and information for your use, to track your activity, to prepopulate online forms with your personal information, and to otherwise facilitate your Site experience. You may block or delete cookies from your internet browser at any time. However, by disabling cookies you may not have access to the entire set of features of the Site.

Advertisements displayed on the Site may also contain cookies or other technologies. Those advertisements may be provided by third party advertising companies, and CTD does not have any control over, and CTD hereby disclaims all responsibility and liability for, the cookies or other technologies used in the advertisements and the use and disclosure of information collected through such advertisement cookies.

Personal Information you Specifically Provide

1. Your Personal Information – When registering for the Site to use one or more of CTD's services, you are required to provide CTD with certain personal information (such as your name, email, postal code, cell phone number, address, date of birth, gender, and certain medical and insurance data) for purposes such as facilitating communications with you, applying for access to certain features or areas of the Site, or carrying out transactions. It is always your choice whether or not to provide personal information. If you choose not to provide requested personal information, you may not be able to access certain features of the Site. Additionally, if you contact CTD, CTD may keep a record of your correspondence.

2. CTD's Use of Your Personal Information – Your personal information is used by healthcare professionals you book your appointments with using the Site. Both CTD, and clinics you have made appointments with or requested information from may use your personal information to contact and correspond with you, to respond to your inquiries, to facilitate and enhance your use of the Site, and as otherwise permitted by law. CTD may develop a confidential personal profile for you to provide the very best service to you. CTD may also use, disclose, and/or publish any of your feedback, reviews or testimonials on a 3rd Party site or on Connect the Doc and re-post that feedback on Connect the Doc and other 3rd Party sites. CTD may also use and/or disclose any information if you have given CTD permission to do so. CTD may share your personal information with its contractors, consultants, and professional advisors to the extent necessary for them to perform services on CTD's behalf and subject to reasonable confidentiality obligations.

3. Disclosure of Your Personal Information – It is CTD's general policy to not voluntarily disclose your personal information to third parties without your consent, except for disclosures required or permitted by law, as permitted pursuant to this Privacy Policy, or reasonably necessary to protect CTD's interests. However, due to the nature of the Internet, Internet technologies, and the existing regulatory environment, CTD cannot ensure that your personal information will not be disclosed in ways not otherwise described in this Privacy Policy.

4. Consent to Disclosure – By using this Site, you signify your consent to the disclosure of your personal information in the following circumstances:

(a) Disclosure to Third Parties Regarding Your Use of the Site – When you post photographs or other materials to the Site, some of your basic personal information may be disclosed as the source of such materials.

(b) Disclosure to Trading Partners, Service Providers and Content Providers – CTD may provide your personal information to certain of its trading partners, service providers and content providers. Before personal information is disclosed, the recipient organization will agree with CTD to protect the privacy of your personal information in a manner that is consistent with this Privacy Policy. While CTD will use all reasonable measures to ensure compliance by such recipient organizations of their respective confidentiality obligations under such agreement with CTD, CTD hereby disclaims all responsibility and liability for any breach by any such recipient organization of its confidential obligations under any such agreement.

CTD may use tracking information to create aggregate reports on user demographics, and Site traffic patterns, analytics, and metrics. These reports may be provided to CTD's advertisers and partners, enabling them to better target their advertisements to the Site's users. None of the tracking information CTD provides them can be connected to the identities or other personal information of individual users. CTD may also link tracking information with personal information. Once CTD makes such a link, all of the linked information is treated as personal information. CTD may use such information to provide you with an enhanced or customized experience. If in any instance CTD intends to use or disclose any personal information collected from users in a manner that would otherwise conflict with the terms of this Privacy Policy, or CTD's Terms of Use, CTD will provide additional privacy information at such instance, which will govern any information so collected.

(c) Law Enforcement and Legal Disclosure – CTD may disclose your personal information to a government institution that has asserted its lawful authority to obtain information, or where CTD has reasonable grounds to believe the information could be useful in the investigation of unlawful activity, or to comply with a subpoena or warrant or an order made by court, person or body with jurisdiction to compel the production of information, or to comply with court rules regarding the production of records and information, or to CTD's legal counsel.

(d) Sale of CTD – CTD may transfer and disclose information about you in connection with a merger, sale or acquisition involving CTD. In such event, CTD will use reasonable efforts to notify you before such information is transferred and becomes subject to a different Privacy Policy.

5. Location of Information – CTD will store and process your personal information on the Site. Except as otherwise provided in this Privacy Policy, only CTD administrators and you can access your personal information. You can add, remove, and edit your personal information by logging into your account.

6. Securing Your Information – CTD uses reasonable security methods to protect the data that reside on our servers. But because no security system is impenetrable, CTD cannot guarantee the security of its servers. It is also possible that information you supply to CTD may be intercepted during transmission.

7. Accessing Your Personal Information – You may request access to your personal information and information about CTD's collection, use and disclosure of that information by contacting CTD directly at legal@connectthedoc.com. Subject to certain exceptions prescribed by law, you will be given reasonable access to your personal information within a reasonable time. CTD attempts to keep its records as accurate and complete as possible.

8. How to Request Deletion of Your Personal Information – You may exercise your rights to access, restrict, or delete your personal information. You can do so by email at legal@connectthedoc.com or by mail at 236-1231 Pacific Blvd, Vancouver, BC V6Z0E2 Canada. In your request, please provide us with your full name, your email address that was registered with your account as well as your phone number that was registered with your account. Once we receive this information, we will have 45 days to acknowledge your request and another 45 days to delete your personal information. Once your information is deleted, we will provide you with an update.

9. Terms of Use Agreement – The CTD Terms of Use Agreement, as may be amended from time to time, contains important provisions, including provisions disclaiming, limiting or excluding CTD's liability for your use of the Site and provisions determining the applicable law and exclusive jurisdiction for the resolution of any disputes that may arise in relation to this Privacy Policy or CTD's collection, use, and disclosure of your personal information. These provisions are incorporated into this Privacy Policy by reference and are of the same force and effect as if they had been reproduced directly in this Privacy Policy.

10. Former Registrants – If you cease to be an active user of the Site, CTD will continue to treat any personal information you have provided to CTD in connection with the Site in accordance with this Privacy Policy, as amended from time to time.

11. Changes to the Privacy Policy – This Privacy Policy may be changed from time to time in CTD's sole discretion and without any prior notice. Your continued use of the Site subsequent to changes to this Privacy Policy will mean and signify your acceptance of such changes.
